Save time at your first appointment by completing your intake paperwork ahead of time. You'll fill everything out securely in your patient portal, with no printing required.
How to complete your intake forms
- Book your first appointment using our online portal or by calling us. You'll receive a confirmation email with a link to your patient portal.
- Log in to your secure patient portal using the link in your confirmation email.
- Complete your intake forms at least 24 hours before your appointment. Forms are reviewed by Dr. Yervasi before your visit so she can make the most of your time together.

Important: protect your health information. Please do not email,
text, or use our general contact form to send health details. All clinical intake is
handled exclusively inside our secure patient portal, which is covered under
a Business Associate Agreement (BAA) and is HIPAA-compliant. This website does not
collect, store, or transmit protected health information (PHI).
